Streamlined Form for Annuity Transfers
A new, separate form makes submitting annuity transfers a bit easier.
You have used our Transfer/Exchange/Rollover form for both life and annuities.
Now, there’s a streamlined form for annuity transfers. This new form only contains transferring options for annuities.
- The new form number is 9685A.
- For life insurance transfers, continue to use the existing Transfer/Exchange/Rollover form 9685.
Reminder:
All transfers need a completed paper form with a wet signature.
Spouse signatures are required in Community Property states (AZ, CA, ID, LA, NM, NV, TX, WA, WI).
