Regulation Update for Pennsylvania Apps
Disclosure statement requirements
The 1458PA: Disclosure Statement is required to be completed with the client at the time of, or prior to, the application being submitted to NLG. This form is a leave behind form for the client.
The 1476: Disclosure Statement Receipt is required by the state of Pennsylvania to be completed and returned to NLG. This form is required to ensure that the 1458PA form was completed for the client.
