Application Kit Updates

The Interest Crediting Strategies Allocations forms 8411 and 8613 have been revised.  They will have a revision date of (0321) and are updated in the following application kits:

8121 LSW Life Insurance Application Kits.  Each kit will have a revision date of (0521).

8121 NL Life Insurance Application Kits.  Each kit will have a revision date of (0521).

20007 Term Conversion Application Kits.  Each kit will have a revision date of (0521).

1441 Policy Change Application Kits. Each kit will have a revision date of (0521).

These are available for ordering beginning April 24^th, 2021.
