Forms and applications are always up-to-date when you use eApp.
With eApp, you are always completing the most up-to-date forms and applications.
Effective March 29, 2024 the life application kits will be revised and available on the Agent Portal under Forms & Materials.

- 8613(0224) – LSW Interest Crediting Strategies Allocations, Cat. No. 53586
- 8411(0224) – NL Interest Crediting Strategies Allocations, Cat. No. 53587
- New York will continue to use 8411NY(0321)
20932(0324)K – Pension Application Kits (previously form number 20713)
- Supplemental Application for Qualified Pension or Profit Sharing Trust Form series 20932(1023) replaces the use of LSW form series 8533(0616) and NL form series 20240(0616), in all states except NY & SC.
