20932(0324)K – Pension Application Kits (previously form number 20713)

• Supplemental Application for Qualified Pension or Profit Sharing Trust Form series 20932(1023) replaces the use of LSW form series 8533(0616) and NL form series 20240(0616), in all states except NY & SC.
