Effective October 30: Life Product Application Forms 8121/8122 Update
With eApp, you’ll automatically have the most current forms.
On October 30, 2021 revised versions of the National Life application forms for Life Products (both the general form, and state specific versions) will be uploaded into eApp and available in the Forms Search tool on the Agent Portal. The framework of the National Life application remains unchanged, with changes primarily centered around the language used in the Q&A process.
What has changed?
Updates to the Base Questions within the General Information & Health History sections of the application:
• Language changes to the base questions
• Removal of some base questions
• Updates to the base question filters in eApp
• Changes to Search Style base questions to use a Picklist/Search style format in eApp
Medical questions will be organized by organ system. The new Picklist/Search Style question format in eApp will offer the impairments listed in the base question as picklist items, but also allows the ability to add additional impairments in a search field (shown below).
Another specific change our team would like to draw our Agents’ attention to is the “$25 premium increase” question within the Banking Section on the application. Historically this question has provided clients the option to “opt in”. It has been updated and now provides the client the ability to “opt out”, meaning the box now needs to be checked if the client would like notification.

How am I affected?
eApp is the easiest way to ensure you always have the most current forms. When you use eApp, you don’t need to worry or change your process… you’ll automatically have the most current form versions.
For written applications, new versions will be available on the Agent Portal, and agents should consider taking a few minutes to acquaint themselves with the revised version following the 10/30 implementation.
What are the transition rules?
For Agents leveraging eApp, the transition is seamless. Applications that are locked (past the first signing ceremony) before 10/30, will be accepted and processed as received, those locked from 10/30 forward will use the new version of the application.
Written applications dated prior to 10/30 will be accepted and processed as received; those dated on or after 10/30 should be submitted using the revised version of the form posted within the Forms Search.
Starting November 30, 2021, all submissions must leverage the revised application (30 day transition).
