Life Application Kit Update!
With eApp, you are always completing the most up-to-date forms and applications
Effective January 27, 2023, the life application kits will be revised and available on the Agent Portal under Forms & Materials
The National Life Insurance Company (NL) and Life Insurance Company of the Southwest (LSW) life application kits (form #8121) will be combined into one multi-company kit. The current catalog numbers for the LSW application kits will be used for the multi company kits and the NL kits will be made obsolete with exception to California (LSW Application Kit – Cat. No. 46529 and NL Application Kit 47464) and New York (NL application Kit 47486).
8121 Life Insurance Application Kits – revision date (0123)
20007 Term Conversion Application Kits – revision date (0123)
1441 Policy Change Application Kits – revision date (0123)
Changes within the application kits:
- The Disclosure Statement for Accelerated Benefits (ABR) forms have been revised.
- NL ABR series are now obsolete and combined as a multi company form using the LSW form numbers 20294, 20299 and 20314
- The Prenotification form 6496NY(1222) has been revised in the New York application kits and no other changes.
No changes have been made to the California application kits.
Note: while the kits are multi-company, some forms are company specific, for example Interest Crediting Strategies. Forms for each company are included in the multi-company kit. Please ensure you are completing the correct company form.
